Analgesics such as paracetamol, ibuprofen, naproxen and codeine are considered to be 'safe', due to low transfer into breast milk.

Antibiotics such as penicillins, cephalosporins and macrolides are considered to be compatible with breastfeeding

promethazine, dexchlorpheniramine and diphenhydramine are considered to be safe.

loratadine and fexofenadine are likely to be safe due to low transfer into milk.

Paracetamol is considered to be safe for use during lactation. The estimated dose received via breast milk is 6% of the maternal dose.

NSAIDs, such as ibuprofen and diclofenac, are considered to be compatible with breastfeeding.

recommend paracetamol as first-line treatment of fever and pain during pregnancy. Codeine or another opioid analgesic can be added to treat more severe pain. NSAID use is contraindicated in the third trimester and alternative analgesics should also be considered in the first trimester.

Piroxicam is excreted into breast milk. A nursing woman, 9 months postpartum, was treated with piroxicam 20 mg/day for 4 months. Milk levels varied between 0.05 and 0.17 g/mL. Based on an ingested volume of 600 mL/day, the investigators estimated the infant would have received a daily dose of about 0.05 mg. However, no drug was detectable in the infant's serum. The concentration of the drug in milk was approximately 1% of the mother's serum levels. These amounts probably do not present a risk to the nursing infant. The American Academy of Pediatrics considers piroxicam to be compatible with breast feeding.
