Simvastatin / ZOCOR® .AL

Simvastatin is a powerful lipid-lowering drug that can decrease low density lipoprotein (LDL) levels by up to 50%. It is used in doses of 5 mg up to 80 mg. Higher doses (160 mg) have been found to be too toxic.

All statins act by inhibiting 3-hydroxy-3-methylglutaryl coenzyme A HMG-CoA reductase. the metabolic pathway responsible for the endogenous production of cholesterol. Statins are more effective than other lipid-regulating drugs at lowering LDL-cholesterol concentration but they are less effective than the fibrates in reducing triglyceride concentration. However, statins reduce cardiovascular disease events and total mortality irrespective of the initial cholesterol concentration.

Interactions
Grapefruit
should not take a simvastatin with Itraconazole, Ketoconazole, Erythromycin, Clarithromycin, Telithromycin, HIV protease inhibitors, or Nefazodone.
Patients taking 10 mg of simvastatin should not take it with Gemfibrozil, Cyclosporine, or Danazol.
If taking 20 mg of simvastatin, do not take it with Amiodarone or Verapamil. Diltiazem should not be taken with 40 mg of simvastatin. These drugs mentioned are CYP3A4 inhibitors which decrease the metabolism of simvastatin

Simvastatin is contraindicated in pregnancy and liver disease.

You should not take Zocor if you are allergic to simvastatin, if you have liver disease, or if you are pregnant or breast-feeding. In rare cases, Zocor can cause a condition that results in the breakdown of skeletal muscle tissue, leading to kidney failure. This condition may be more likely to occur in older adults and in people who have kidney disease or poorly controlled hypothyroidism (underactive thyroid).

These drugs should not be used while you are taking Zocor:
cyclosporine (Gengraf, Neoral, Sandimmune);
danazol (Danocrine);
gemfibrozil (Lopid);
nefazodone (an antidepressant);
the antibiotics clarithromycin (Biaxin), erythromycin (E.E.S., EryPed, Ery-Tab, Erythrocin, Pediazole), and telithromycin (Ketek);
the antifungal medications itraconazole (Sporanox), ketoconazole (Nizoral), and posaconazole (Noxafil);
hepatitis C medications such as boceprevir (Victrelis) or telaprevir (Incivek); orthe HIV/AIDS medications atazanavir (Reyataz), darunavir (Prezista), fosamprenavir (Lexiva), indinavir (Crixivan), nelfinavir (Viracept), ritonavir (Kaletra, Norvir), saquinavir (Fortovase), or tipranavir (Aptivus).

FDA pregnancy category X. Zocor can harm an unborn baby or cause birth defects.

To make sure you can safely take Zocor, tell your doctor if you have any of these other conditions:
history of liver disease;
history of kidney disease;
diabetes;
a thyroid disorder; or
if you drink more than 2 alcoholic beverages daily.

Experts suggest that the sleep disturbance may be due to the fact that simvastatin is capable of crossing the brain. Unlike other cholesterol-lowering drugs, simvastatin is fat soluble which allows it to readily cross into the brain and affect the cell membranes. The brain regulates sleeping pattern, any disruption or alteration in its normal physiology can result in adverse effects on sleep quality.

The researchers further stressed that the disruption in sleep pattern can affect the patient’s quality of life and have negative effects on health that includes weight gain and insulin resistance. Patients who experienced sleep problems while taking simvastatin are at increased risk of gaining extra weight compared to others. Therefore, if you are on simvastatin therapy and develop sleep problems, you should consult your healthcare provider

If you are taking cholesterol-lowering meds, you may want to have a discussion with your MD about having a VAP cholesterol screening to look for the BAD-sized LDL particles. Remember, your body requires cholesterol to make all of your sex hormones like estrogen, progesterone and testosterone. Without adequate levels of cholesterol, you will suffer depletion of these vital hormones, as well.

it plans to add the diabetes-risk language to the "warnings and precautions" sections of labeling for the statin drugs. warning that patients taking the drugs may face a "small increased risk" of higher blood sugar levels and of being diagnosed with diabetes.


The drugs that will get the warning include top-selling brand names such as Lipitor, Lescol, Pravachol, Crestor, Mevacor, Altoprev, Livalo and Zocor.

Many patients who suffered adverse events have filed Zocor lawsuits, accusing manufacturer Merck of failing to warn consumers of the risks of developing Zocor diabetes or muscle injury.

The FDA has recently warned physicians to avoid prescribing 80 milligram dosages of Zocor because of the heightened risk of muscle injury associated with that dosage, the highest previously dose approved by the health agency. According to the FDA, Zocor side effects like myopathy –muscle pain and weakness that can lead to rhabdomyolysis and kidney failure – are a risk at any dosage, but particularly in extremely high dosages.
